fix: make content-grep pattern a positional argument
muse content-grep "TODO" is now the interface. The --pattern/-p flag was a gitism — every grep-like tool (grep, rg, ag, ack) takes the pattern positionally. Removes the flag entirely; no backward-compat shim.
