fix(hub): narrow hosted enrich request and improve policy button clarity

Keep hosted Enrich close to the last known good runtime shape while preserving suggested frontmatter output, and log async review-hints failures instead of swallowing them. Also make the proposal policy save action read like a real button and return JSON 413s for oversized Hub requests.
