fix(icp): identity stable migration after V5 so repeat dfx deploy succeeds

Motoko M0216 rejected upgrades when on-chain storage was already StableStorage (V5) but the actor hook still expected StableStorageV4. After the one-time V4→V5 upgrade, mainnet matches full ProposalRecord; the migration input must be StableStorage with an identity transform.

Rename unused helpers with underscore prefix; document V4 one-shot path in Migration.mo and PROPOSAL-ENRICH-EXTENSION-PLAN; update verify script.
