fix(gateway): disable express-rate-limit validations on MCP OAuth routes

Per-key validate overrides still hit ERR_ERL_UNEXPECTED_X_FORWARDED_FOR behind Nginx with the SDK auth router. Use validate: false for those limiters only; rate limits remain; Nginx edge limits still recommended.
