fix(gateway): relax all MCP OAuth rate-limit proxy validations; doc PM2 path

ERR_ERL_UNEXPECTED_X_FORWARDED_FOR persisted with only xForwardedForHeader disabled. Align validate with express-rate-limit v8 (trustProxy, forwarded).

DEPLOY-HOSTED: warn when PM2 uses /opt/knowtation but npm ci ran only in ~.

test: phase3 §3.4b asserts MCP OAuth SDK rateLimit validate block.
