Phase 13: Hub roles (data/hub_roles.json), JWT role, requireRole, setup GET/POST

- hub/roles.mjs: loadRoleMap, getRole, readRolesObject, writeRolesFile - server: issueToken includes role (admin when no roles file, else from store) - effectiveRole/requireRole; protect notes, index, proposals, approve/discard, setup POST, roles, vault sync - GET/POST /api/v1/setup, GET /api/v1/settings with role; hub README OAuth and roles
