docs(phase17): add note rendering & media model section

Documents decision to: - Enable markdown rendering for note bodies (17A) using existing marked+DOMPurify pipeline - Adopt "pointer, not payload" media model — assets live in user's own storage (Drive, GitHub, CDN) - Document public vs. private URL rendering behavior and agent access pattern - Defer native image upload to a future phase - Re-sequence build order: 17A markdown render → 17B tier selector → 17C op metering → 17D pack labels → 17E tokens (deferred)
