fix(gateway): paginate notes when merging vault-image resources into MCP list

Scan GET /api/v1/notes in pages until image URIs cap, end of notes, or HOSTED_IMAGE_LIST_MAX_NOTES_SCANNED (5000). Ensures resources/list can surface embedded images beyond the first canister page for large vaults.

Adds R3 test with paginated list mock.
