fix(hosted-mcp): canonical image URI vault-image/{notePath}/{index}

Avoid knowtation://hosted/vault/.../note.md/image/n overlapping the greedy vault/{+path} template; some MCP clients return resource not found for reads.

- Advertise knowtation://hosted/vault-image/{+notePath}/{index} in resources/list - Keep legacy …/vault/…/image/n via hosted-vault-note regex + test - Docs: parity matrix + handoff table
