docs(kd-5a): session prep — paste file + branch hygiene

Create feat/kd-5a from feat/kd-oq so the next session inherits KD-OQ governance without branching from main. Add MUSEHUB-OVERSEER-HANDOVER-PASTE.txt (one-file copy for fresh chats) and document branch hygiene in the handover VCS table and KD-5a paste block.
