fix: deduplicate fetch mpack branch tips

Fixes the clone cache miss/rebuild loop when multiple branch refs share one commit ID. Deduplicates want/have IDs at fetch entry and deduplicates live/payload prebuild tips while preserving order.

Validation on isolated local PostgreSQL database musehub_test:

  • python -m py_compile musehub/services/musehub_wire_fetch.py tests/test_fetch_mpack_prebuild.py: exit 0
  • duplicate-head regressions: 2 passed, 11 deselected (1.51s)
  • tests/test_fetch_mpack_prebuild.py: 13 passed (7.01s)

No Movie source or frozen governance artifacts changed. Requesting Gabriel review; this proposal must not be self-merged or deployed before approval.
