chore: drop dead musehub_stream_rejections table (MWP cleanup)

The streaming wire protocol (MWP) is gone. The musehub_stream_rejections table was its push-rejection audit trail — never had a single insert in production. Delete the ORM class and add migration 0064 to drop the table and its two indexes.
