fix: uniform MCP tool naming — verb_noun pattern throughout

- get → read for all single-item reads (context, commit, domain, view, prompt, symbol) - submit_proposal_review → create_proposal_review - patch_repo_settings → update_repo - set_issue_labels → update_issue_labels - Add missing verb prefixes: proposal_risk → read_proposal_risk, etc. - Coord tools reordered to verb-first: coord_reserve → create_coord_reservation, etc. - 31 tool renames; executor functions renamed consistently
