fix(symbols): prefer symbol-level addresses in coupling partners
Bumped SQL limit to 60 so filtering doesn't starve the final 20. After fetch, any bare file.py entry is suppressed when a file.py::Symbol sibling is present — the symbol is strictly more informative. Files with no symbol-level entries still appear as fallback.
