feat(intel): BlastRiskProvider SQL implementation, 30/30 phase 1 tests green

- Replace muse-CLI subprocess with pure SQL derivation from musehub_symbol_intel - Add _compute_risk_score() and _risk_tier() helpers with 4-weight formula - Upsert into musehub_intel_blast_risk on (repo_id, address) - Use populate_existing=True on SELECT to bypass stale ORM identity map on reruns - Fix state-integrity tests: capture repo_id before expire_all, avoid lazy-load trap
