diag(wall19): per-frame elapsed+gap timing in iter_wire_frames_grpc
Add time.monotonic() to each frame log: elapsed since stream start and gap since previous frame. Diagnoses Wall 19 — determines if frame 116 truncation is time-based (large gap) or size-based (gap normal). Also logs total elapsed at EOF.
