fix: add .env.example to unblock 12 skipped environment tests

All 12 skipped tests in test_environments_section71.py were gated on _skip_no_env_example (file missing). .env.example now satisfies every assertion: MUSE_ENV, DB_PASSWORD, DEBUG, CORS_ORIGINS, WEBHOOK_SECRET_KEY, RUNNER_TOKEN, staging/production guidance, secret generation commands, no real secrets embedded.
