fix(wire): drop gRPC frame unwrapping from push stream

Wall 16: client no longer wraps frames in gRPC envelopes. Switch iter_wire_frames_grpc -> iter_wire_frames in wire_push_stream. Remove debug instrumentation from push/stream route handler.
