feat: calibrate staging gates for l/xl sizes from observed runs

- push/l/presign: 5597ms observed → 12s gate - fetch/l, pull/l, clone/l: stream path, 845-2152ms → 2-5s gates - xl: extrapolated from l at observed 4 MB/s throughput - curl PUT via Homebrew OpenSSL to work around macOS LibreSSL + R2 TLS issue - retry with 12s backoff for R2 rate limiting on sequential large uploads
