fix(phase5): add statement_timeout=60s to DB pool connect_args
Per-statement timeout catches runaway queries without killing healthy per-batch push commits (which complete in milliseconds). Adds P5-3 structural test to enforce the setting alongside tcp_keepalives_idle.
Closes issue #41 Phase 5 gap identified in review.
