docs: track issue #132's plan doc in-repo, not /tmp

Formalizing a pattern already partially in place (docs/issues/mwp-mvp-master.md, docs/issues/release-prune-pep440-sort-plan.md, etc.): local .md source files for issue/mist bodies live in the repo under docs/issues/ for as long as the issue/mist is active, and get 'muse rm'd once resolved -- versioned, survives reboots, discoverable, and never becomes permanent clutter.
