feat(coupling): Phase 3 — /intel/coupling route + template
Route: GET /{owner}/{repo_slug}/intel/coupling - min_co filter (default 2), top page size (25/50/100/200, default 50) - bar_pct normalised against top pair; heat_modifier from _cp_heat() - short_a/short_b from _cp_short() — last 2 path components for display
Template: intel_coupling.html - intel-page-header pattern matching stable/dead/entangle pages - intel-meta-bar: pairs count, ref, built date - Filter form: min_co input + top select + Apply - .cp-list with .cp-pair-row: file_a ↔ file_b inline, count right, heat bar below - Empty state with icon fallback
