feat: remove Cloudflare Worker push path (pack_origin / object-pack)

Deletes _post_object_pack, _WORKER_CHUNK, _ObjectPackResult, pack_origin from RemoteInfo and RemoteEntry/RemoteConfig, get_remote_pack_origin, and all pack_origin handling in transport.py and config.py.

The push function signature no longer takes pack_origin. Two paths remain: stream (small) and presigned R2 (large).
