docs: Wall 19 — stream truncation findings from SSM logs; decompose to atomic units
W17-A3 complete: SSM Docker logs confirm auth works, ASGI receives bytes, 115 frames process cleanly. Two failure modes identified: - 19-A: frame 116 (6KB O-frame) truncated to 2430 bytes within batch 1 - 19-B: batch 2 RST_STREAM before ASGI ENTER fires between batches
Wall 17 marked diagnosed (superseded by Wall 19). Wall 19 added with full decomposition: W19-A0 through W19-A6, ranked by diagnostic value. Most atomic next step: per-frame index+size logging in iter_wire_frames_grpc (W19-A1).
