refactor: [t] plan_merge, predict, prune — EnvelopeJson TypedDicts
plan_merge.py: add _PlanMergeErrorJson, _PlanMergeItemJson, _PlanMergeJson (EnvelopeJson); drop schema_version + __version__ import; thread elapsed through _err(); fix 3 _err call sites + 1 JSON output. predict.py: _PredictJson, _ExplainJson → EnvelopeJson; fix 2 output sites. prune.py: add _PruneResultBase(EnvelopeJson) + _PruneResultJson(total=False) for optional candidates field; fix 2 output sites.
