docs(mwp4): mark MWP-4 COMPLETE in master tracker, flip RC-4 criterion

musehub#109 closed. Three-layer fix: created_at stagger (Phase 1), claim-layer NOT EXISTS barrier (Phase 2), narrow FetchNotIndexedError swallow (Phase 3). 12 MWP4 tests + 32 adjacent tests green.

RC-4 acceptance criterion: 'open → MWP-4' → 'COMPLETE fixed by MWP-4'.
