refactor: replace remaining string concatenation with f-strings

entangle.py, piano_roll.py, type_cmd.py, callgraph_cache.py, implicit_edge_cache.py, symbol_cache.py, _invariants.py, code_stage.py, identity/plugin.py — all remaining + patterns converted to f-string interpolation.
