refactor: extract bridge hooks to muse.core.bridge.hooks (issue #14 Phase 2)

Move BridgeHook, BridgeHooks, load_bridge_hooks, run_hook, run_hooks out of bridge.py into a focused muse.core.bridge.hooks module. bridge.py now imports from there. All 117 bridge tests green.
