refactor: extract harmony/shelf bridge to muse.core.bridge.harmony_shelf (issue #14 Phase 5)

Move import_rerere_to_harmony, export_harmony_to_rerere, import_stashes_to_shelf, and export_shelves_to_stash out of bridge.py into a focused harmony_shelf module. Update importer.py lazy imports to point to the new location. 2355→1301 lines.
