docs: replace UUID terminology with content-addressed ID language
Update all markdown docs to match the source refactor: - crdt-music-rga.md, crdt-midi-rga.md: op_id described as random hex, not UUID4 - entity-identity.md: entity_id is content-addressed, not UUID4 - op-log.md: op_id field is sha256: content-addressed ID - utility-reference.md: coordination/task IDs now sha256:, table and Pattern 4/7 updated - agent-guide.md: <uuid> placeholders → <id> in coord/hub commands; harmony audit path updated - README.md: repo.json identity field updated - reference/cli-tiers.md, remotes.md: repo_id examples use sha256:<hex> - reference/code-domain.md: reservation/intent schemas use sha256:<hex> - guide/crdt-reference.md: OR-Set token described as random hex
