docs: document I2 grace-period hub policy (musehub#226)

Updates the identity plugin's I2 docstring (plugin.py) and the domain-extensibility doc's I2 bullet to reflect the resolved hub-level policy: an orphaned identity node (no path to any human root) warns for a 30-day grace period, then becomes a hard error at push time if still orphaned past that window. Companion change to musehub's graph/push_validator.py implementation on the musehub side of this ticket.

Confirmed muse-side regression: 85 passed across test_identity_plugin_invariant_docs.py and the identity-domain test suite.
