docs(agents): branch discipline — constraint is on leaving, not starting
Add full Branch Discipline section with lifecycle steps, stash workflow, task teardown sequence, and enforcement protocol. Fix Phase 0 pre-flight comment. The rule is 'commit or stash before switching away', not 'start clean before branching'.
