fix(muse-adapter): stage via muse code add in commit_feature

Live GSW land-b dogfood: both Muse adapters staged with bare 'muse add', but Muse 0.2.x has no top-level add subcommand (staging is 'muse code add', as the KH2 remediation string already documents). First live --write failed at Step E. Fix both adapters; BranchStateRunner now rejects bare add like the live binary; GSW unit stubs + code-add assertion added. Targeted suites 289 green.
