feat(governance-sync): regenerate NEXT + paste-ready from queue

Close the 9A-5 gap: ok governance-sync now rebuilds next-session and paste-ready-prompt from an unambiguous open queue row, fail-closed on ambiguity, with glance aligned and git-only proven Muse-free.
