NXP-a: freeze NEXT provenance + board identity activation (pass NXP-r3)

Closes the live incident where two repos injected byte-identical '## CURRENT NEXT — paste this' headings into one chat with no attribution.

N1 visible provenance line on ok next human stdout (narrow supersede of ONS §ONS.5.4; heading stays line 1, fence body bytes untouched) N2 additive --json repo_name/repo_root/read_at N3 ok workspace check-next stops being a silent exit-0 success N4 bare-board-name advisory in ok status (warn, never a gate) N5 hook enablement sequenced behind NXP-b — hooks are NOT disabled

Roadmap: NXP-a DONE, NXP-b QUEUED; auto-enable-hooks backlog row blocked by N5. Handover NEXT regenerated from roadmap (SD-17).
