fix(hosted-mcp): relate titles from body # heading or path stem

Many hosted notes omit title in JSON frontmatter; title lives in the first markdown ATX line. displayTitleFromHostedNote: frontmatter title, then body heading, then filename stem. Neighbor hydration uses it.
