docs(phase12): add PHASE12-BLOCKCHAIN-PLAN.md, update blockchain and implementation docs

- docs/PHASE12-BLOCKCHAIN-PLAN.md: full build plan with field table, framing (agent wallet records), scope breakdown (4A–4E), example note, and ops notes - docs/BLOCKCHAIN-AND-AGENT-PAYMENTS.md: updated to reflect Phase 12 is now active; expanded frontmatter field table (added amount, currency, direction, confirmed_at, block_height); added payment_status conventions and recommended tag list; updated summary table - docs/IMPLEMENTATION-PLAN.md: moved Phase 12 from 'Later/Deferred' to 'Active' row with branch name and list of features; MCP D2/D3 remains deferred separately
