feat(mcp): add bootstrap prime resources for stdio and hosted MCP

Expose knowtation://prime (self-hosted) and knowtation://hosted/prime with schema knowtation.prime/v1, token-layer hints, and hosted prompt names per role. Add regression tests and parity matrix row.
