feat(2F-b-d-kn-d): enable attach gate in dev/staging only

Flip MEDIA_ATTACH_ENABLED via staging seed + policy fixture; extend live smoke for attach propose path. External-link gate unchanged (KN-MD-4). §16.2 KA-* passed; 22/22 tests; live smoke 7/7. Unblocks Scooling 2F-b-d-f.
