chore(gateway): log canister 4xx on GET note; add NOTE-LOAD-404-FORENSIC

- Gateway: log [gateway] canister GET note: status url when canister returns 4xx so Netlify function logs show exact URL sent and confirm 404 from canister. - docs/NOTE-LOAD-404-FORENSIC.md: architecture, what we tried, deploy requirements (canister + gateway), and where to check logs.
