fix(hosted): evaluators ignore hub_scope project/folder limits

Bridge resolveHostedBridgeContext returned per-user scope for evaluators, so a scoped row (e.g. projects: [Project-1]) hid most notes. Evaluators need full vault visibility within allowed_vault_ids to review proposals.

Gateway hosted-context inherits scope: null; vault allowlist unchanged.
