feat(hub): server-set note provenance; git backup push/upstream fixes

- Add lib/hub-provenance.mjs: merge knowtation_editor, knowtation_edited_at, author_kind; strip client forgeries of reserved keys - Wire into POST /notes, /capture, /import (provenance pass), proposal approve; store proposed_by on proposals - vault-git-sync: align origin with hub remote; git push -u origin HEAD; ls-remote verify; spawnSync for reliable stderr; safeRevParse - Hub UI: Initialize Git vault endpoint handling, api HTML error hint - test: hub-provenance unit tests; config test env isolation for fixtures - Docs: hub/README provenance + signed commits ops; HUB-API; LOCAL-DEV-TEST-GUIDE
