feat(import): docx source type and importer

Add mammoth-based DOCX→Markdown importer (inbox/imports/docx/), register docx in import map and canonical source types, Hub modal option + copy, hosted MCP import parity, golden and bridge integration tests with minimal OOXML fixture, and documentation updates (IMPORT-SOURCES, HUB-API, openapi, phases, parity).
