fix(bridge): redirect to Hub path with correct query params after Connect GitHub

- Add HUB_UI_PATH (default /hub) so redirects land on knowtation.store/hub/ not root - Use ?github_connected=1 on success and ?github_connect_error=... on errors (Hub expects these) - Document HUB_UI_PATH in bridge README
