test+config: KNOWTATION_VAULT_PATH overrides hub_setup vault_path

hub_setup.yaml was applied after env, so KNOWTATION_VAULT_PATH (e.g. CLI tests) lost to Hub-written vault_path. Skip hub_setup vault_path when the env var is set; merge vault.git unchanged.

Stabilize parallel test runs: pin fixture vault in runListNotes; move hub-setup read/write tests to fixtures/hub-setup-test-data so they do not race config tests on fixtures/data/hub_setup.yaml. Add regression coverage; gitignore the hub-setup test data dir. Update IMPLEMENTATION-PLAN status.
