fix(hub): note load 404 + close modal after create

Canister: - Decode percent-encoded path on GET /api/v1/notes/:path so lookup matches POST-stored keys (e.g. inbox%2Fnote.md -> inbox/note.md). - Add decodePercentEncoded and charToHex in main.mo.

Hub UI: - Close Add-to-vault modal after successful Quick save and Full create.
