feat(gateway): register hosted MCP relate tool

Wire relate for hosted MCP: canister source note read, bridge POST /api/v1/search (semantic), self-path exclusion, neighbor title hydration. Document bridge vs local Voyage query/document embedding gap.

Tests: mcp-hosted-relate.test.mjs; golden tool lists; checklist copy.
