feat(hub): Muse thin bridge Option C (approve external_ref, admin proxy)

- Add lib/muse-thin-bridge.mjs: MUSE_URL/MUSE_API_KEY, lineage GET, proxy allowlist - Node Hub: resolve ref on approve, persist via proposals-store, audit detail, GET /api/v1/operator/muse/proxy - Gateway: merge ref into approve body before canister; same operator proxy route - Canister: persist external_ref on approve; include in JSON response - Docs: MUSE-THIN-BRIDGE.md, HUB-API/openapi/lifecycle/AGENT-INTEGRATION/IMPLEMENTATION-PLAN/DEPLOY-HOSTED - Tests: muse-thin-bridge unit, proposal store, gateway approve body integration

Deferred: MCP history stub, CLI muse subcommand (per IMPLEMENTATION-PLAN).
