fix(gateway): avoid Netlify crash on import.meta.url; Hub proposals + agent env UX

- lib: resolve __dirname safely in hub-proposal-rubric and hub-proposal-review-triggers when bundled (import.meta.url undefined), fixing gateway cold-start TypeError on Netlify. - Hub: Settings → Integrations copy for KNOWTATION_HUB_URL, token, vault; Agents button renamed to Copy embedding env; Suggested/Discarded tabs refresh proposal lists on open. - Hub: New proposal modal and Propose change on note detail; How-to copy aligned. - docs: AGENT-INTEGRATION.md — curl example, copy-button flow, human proposal paths.
