feat(mcp): Phase G scope (initialize instructions + client roots log)

- Add mcp/server-instructions.mjs: plain-language + file:// URIs for vault/data_dir (multi-vault lines) - createKnowtationMcpServer: pass instructions; after initialized, roots/list + structured log when supported - docs: MCP-PHASE-G.md; backlog/IMPLEMENTATION-PLAN/issue-1 aligned with MCP spec (client roots) - Plain-language intros on backlog, AGENT-INTEGRATION, AGENT-ORCHESTRATION, phase docs A–E,H; fix register links to create-server.mjs
